I, the applicant on behalf of myself, members of my family, executors, administrators and assigns, hereby forever release from liability, discharge from responsibility and hold harmless Hollister House of Grappling, its owners(s), employees, representatives and agents for any injury, loss or damage to my person or property, howsoever caused, arising from or in connection with my, or the person under my guardianship, taking part in classes, self-defense workshops, seminars and/or other activities and not withstanding that the same may have been contributed to or occasioned by negligence of Hollister House of Grappling, it’s owner(s), employees, representatives and agents. I understand that participants must supply their own protective equipment. I, the undersigned acknowledge that the waiver was read, understood & agree to abide by it.
